Our Mission is to provide professional guidance, support, education, and services to elders and their families.
 Maintaining quality of life for an elderly relative is a difficult prospect; given the challenges of time, distance, and schedules. Care Resources' experienced professional Geriatric Care Management staff can provide one-to-one assistance and direction for your loved one and peace of mind for you. We recognize that each set of needs is unique and we strive to bring a personalized touch to every aspect of our service.

 Families rely on Care Resources to provide a trusted link to the seniors we serve. Our professional and credentialed Geriatric Care Managers have comprehensive training and expertise in elder-related issues that will ensure loved ones attain or maintain their highest possible level of autonomy in a safe and appropriate environment. We begin with a thorough assessment to identify needs to develop a personalized care plan designed to maximize strengths. As a liaison for families and other involved professionals, we ensure the wishes of the client and family are honored whenever possible.
Personal Care Plans
 Our assessment results in a personal plan of care, which once approved, will chart out client services. Ancillary staff and additional service providers are educated in client needs; which supports improved continuity. Care Resources can help give peace of mind and assurance to families that their loved one’s needs are important.
