• Provide an interface with physicians and other healthcare providers
• Monitor client’s adjustment with medical directives including medication management
• Coordinate services with attorneys,bank trustees, insurance providers and other financial/legal professionals
• Arrange and monitor outside service providers to ensure client’s needs are properly met
• Assist in applying for Medicare, Medicaid, VA, and other benefits
• Assist with the selection of and relocation to a retirement community, assisted living facility, nursing home or rehabilitation facility
• Assist family members to determine when home care is no longer safe or appropriate
• Transportation and escort to appointments
• Assist with bill paying and insurance filing
• Provide socialization, shopping, errands, housekeeping, and other services
• Coordinate the provision of care in private home or alternative care facility locations
• Crisis response 24-hours / 7 days a week
